v      For those HMO patients sent from the emergency room for early morning clinic appointments (7:30 a.m. 躱5 a.m.) it is your responsibility to contact your primary care physician as soon as they are available to obtain a referral for your emergent visit.

v      If you have an Insurance company with which UBOA does not participate, it is important to remember that your carrier may require that you pay additional charges (deductibles and Co-insurance) for going to a Non Participating provider. You will be billed for these amounts.

v      If you have no insurance, please be prepared to make a partial payment for all visits.

v      The following insurance carriers have very restrictive procedures and the ER will refer you to a Primary Care provider or participating provider. Upper Bucks Orthopaedics does not participate with these carriers.

  • Health Partners

  • AmeriChoice

  • TriCare Prime

v      Many Orthopaedic problems will be managed by your primary care provider who will refer you if necessary; please follow his instructions.

v      If you are located outside the geographic area you may be referred to an orthopaedic surgeon within your area.  This is for your follow-up convenience.

v      CO-PAYS ARE REQUIRED AT THE TIME OF SERVICE. THERE IS A $10 CHARGE IF CO-PAYMENT IS NOT PAID AT THE TIME OF SERVICE.
